Intramedullary fixation of proximal tibial fractures

چکیده
is challenging owing to the relatively high chance of displacement during nail insertion. The stability of the proximal fixation is inferior because of the wider diameter of the intramedullary canal in the metaphysis. This has led to the use of Poller screws for additional blocking during intramedullary fixation of proximal or distal tibial fractures.1 The effects of Poller screw is two-fold. First, fracture reduction is better maintained during nail insertion, which reduces the chance of malunion. Second, the stability achieved by additional Poller screws enables quicker fracture union. Stability can be further enhanced by modifying nail design and locking options,2 such as increasing the number of locking screws inserted into the metaphyseal fragment, and using angularly stable locking screws that diminish the amount of nail wobble in the screw hole.3 Commentary: Intramedullary nailing supplemented with Poller screws for proximal tibial fractures

Extra-articular proximal tibial fractures: nail or plate?
The surgical goals for treating proximal tibial fractures are to restore articular congruity, the mechanical axis, and knee motion while avoiding soft-tissue complications. The fracture pattern should be correctly identified and understood. For fractures with minimal intra-articular extension, fracture fixation with an intramedullary nail can decrease the risk of infection because it uses a sma...

Background and purpose: Ëxternal fixation is currently considered as a standard treatment for open tibial fractures however, due to the long-term hospitalization and some other complications, some researchers have proposed intramedullary nailing as the alternative surgical treatment. This study was conducted to compare the treatment of open tibial shaft fractures by external fixation and int...
متن کاملFinite element analysis of intramedullary nailing and double locking plate for treating extra-articular proximal tibial fractures
BACKGROUND Proximal tibia fractures are one of the most familiar fractures. Surgical approaches are usually needed for anatomical reduction. However, no single treatment method has been widely established as the standard care. Our present study aims to compare the stress and stability of intramedullary nails (IMN) fixation and double locking plate (DLP) fixation in the treatment of extra-articu...
متن کاملIntramedullary nailing and angulation prevention in distal metaphyseal tibial fractures.
Intramedullary nailing, which is preferred in tibial diaphyseal fractures, is also frequently used in distal third tibial fractures. Various angular deformities, including varus/valgus deformity, may be observed during postintramedullary nailing. Orthopedic surgeons use several methods to prevent this problem. In this study, at least 2 static locking screws were placed proximal and distal to the...
متن کاملTreatment of Proximal Femur Osteomyelitis Occurred after Proximal Femoral Nail Antirotation Fixation, with Antibiotic Cement-coated Tibia Intramedullary Nail: A Case Report
Antibiotic cement-coated intramedullary nails maintain a locally high antibiotic concentration while contributing to bone stability. We present a case of femoral subtrochanteric fracture in a patient with an infected nonunion who was successfully treated for an infection and nonunion using an antibiotic cement-coated tibial intramedullary nail. A 79-year-old woman with a right femoral subtrocha...
